I accept insurance payments from several different companies. Please check with me on which insurance company you use. Even if your insurance company is not listed below, you may be able to receive reimbursement from your insurance company. I would be considered an out-of-network provider. If you use insurance, you are responsible for the co-payment. If no insurance is available, I accept MasterCard, VISA, cash and checks. Payment is due at the time of service.
